Primary Care Support England are currently sending out letters to registered patients asking to confirm your registration details. We believe this is to patients that have not been seen in the surgery for a number of months.
They are asking if your address is correct and then the completed form has to be sent back in the pre paid envelope.
If you do not respond they are removing patients from the practice list so it is very important if you receive this letter that you reply.
